CRITICAL LEGAL REQUIREMENT: The institution is legally mandated by federal law to report any Subject Invention supported by federal funds (including those received as subcontractor to another institution). Failure to link a federal grant can result in the loss of patent rights.  

Complete questions A & B, indicating if federal funding and then other funding, supported this invention.  Listing of the funding source is a lookup of agreements already in the Insight system.  To link an agreement:

1. Click the “Add Agreement” link.  
2. Search for an agreement by any of the presented fields: Agreement #, Fund #, Award # or PI Name.  
3. Click the checkbox next to the appropriate record(s).
4. Click the “Apply” button.  
5. The popup will close, and the record will be listed. 

Question C asks about additional funding from outside MGB, including Shriners, CIMIT and HHMI. 

Question D is for any other support used in the generation of this disclosure, but which does not have funding associated to it.  This can include CDAs, MTAs and DUAs. The lookup for these agreements is similar to funded records but the search options are limited to Agreement # or PI Name.
